Yes I would like my child to participate in the ABC Music & Me Program at
I agree to pay at the beginning of each 4 week, 3 month or 6 month session. I understand that it is my responsibility to inform the center director when my child will no longer participate each session until I inform the center of my child's departure from the program in writing.
I understand that is my responsibility to ensure that my child attends all scheduled lesson. I acknowledge that if my child misses a lesson, my child will not be entitled to a make-up lesson nor will I be entitled to a credit or a refund.
